Australian-first platform connects RMMRs to My Health Record in real time
Built for the future of aged care, Meditrax’s new platform saves time, improves medication visibility, and supports connected clinical care.

Meditrax introduces a first-of-its-kind technology platform that enables Residential Medication Management Reviews (RMMRs) and Home Medication Reviews (HMRs) to be uploaded directly to My Health Record. An important breakthrough that addresses one of health care’s most persistent challenges, continuity of care.
Purpose-built for aged, home and disability care settings, the cloud-based platform is the first of its kind. It provides faster access to critical medication information and helps teams collaborate more effectively across the care journey.

Today, each care provider acts in silos. Our new platform connects the care ecosystem including a dashboard for facilities and consulting pharmacists, enabling faster clinical reviews, visibility across MAC discussions, and better oversight of care trends.

Meditrax has integrated with Webstercare’s Electronic National Residential Medication chart (eNRMC platform), MedCare™ to enable seamless integration, reducing duplication of data and improving the flow of information between our clinical pharmacists and facilities. Design was influenced by direct input from aged care clinicians and stakeholders.

Built with providers, for providers
Meditrax paired up with digital health company Nimbly to modernise its platform, transforming MRS into a scalable, cloud-native solution.
The new system supports offline access, enables seamless integration with My Health Record, and delivers real-time insights through an AI-powered data bot called Pulse.
Early results show clinical pharmacists reduce 30% of their day-to-day workflow.

The platform is currently being piloted in select aged care facilities and will be formally launched at the Meditrax Care Practices Forum this September.
Looking ahead, the platform will enable Meditrax to publish annual de-identified benchmarking reports on medication safety and clinical governance. Helping providers measure performance and support continuous quality improvement.
